Dr. Brian Graham
Brian H. Graham MD, CHMD. In 1991, Dr. Graham completed his 3 year residency in Family Practice at the San Bernardino County Hospital in Southern California. He spent the next 10 years working with various medical groups in Hawaii, Northern California, and Southern California. In 2000 he moved to San Diego to join the Call Doctor Medical Group. For the next 5 years he did house calls for home bound patients. For the past 15 years he has served as medical director for several home care, home health, and hospice agencies. During these past 15 years he has focused his practice on end of life care. He works closely with the whole hospice team of nurses, social workers, and spiritual counsellors. He continues to bring hospice care to the patient and family where ever they are, home, board and care, nursing homes, and hospitals.
