Terri Cady, Community Education and Outreach Director for Disability Network/Mid-Michigan, has worked to create independent facilitation as a real option for person-centered planning (PCP) throughout Michigan since 2005. She has developed, implemented and monitored contracts for facilitating PCP in six counties. Terri is a PCP trainer and an advocate at the local and state levels for long-term care options and accessibility for all citizens. From the grassroots level, she has worked with Veterans Affairs, Community Mental Health, Michigan Rehabilitation Services, Michigan Works!, Michigan Office of Services to the Aging, and others and continues to develop opportunities to enhance service

Wendi Middleton is Director of the Program and Partnership Development Division at the Michigan Office of Services to the Aging (OSA). Wendi leads OSA’s effort to integrate person-centered planning (PCP) into the aging network and works developing Michigan’s emerging Aging and Disability Resource Collaboratives (ADRC’S) and ADRC standards, specifically leading the team creating Options Counseling training and certification.
